Camper Health Form 1
This health history form (to be completed by the camper's parent/guardian) must be completed and submitted each year. The form must be mailed in time to arrive at Lakeside at least two weeks prior to the first day of camp.

Camper Health Form 2
This form is to be completed by the camper's physician. Bring this form (along with a completed copy of Form 1) to your doctor. State law and ACA standards require that Lakeside has on file a record of a physical exam conducted within 24 months of the camper's arrival at camp.

Scholarship Application
To request financial assistance for your camp tuition, complete this form and submit it to Lakeside as soon as possible. Scholarships are generally need-based and award decisions are based on a variety of factors (including things such as family size, household income, tuition assistance you expect from other sources, etc.). Scholarship amounts are also influenced by the funds available in Lakeside's scholarship fund.

Staff Application Packet
If you wish to apply for a summer staff position, you may download the staff application packet. The packet includes a Staff Application Info sheet, the Application Form, a Reference Form (you will need 3 copies), the Background Investigation Forms and a staff health form. If you are a returning staff member, you do not need to submit written references (however you must still list references on your application form). The health form is not required as part of the application process, but you must have a physical (within 24 months before camp) if you are hired. All staff members and volunteers must submit an updated health form upon arrival at camp.

Summer Staff Job Descriptions
These are general descriptions of the various summer staff positions. When you have received your employment agreement, please locate and review the job description for your position(s).

Staff Health Form
All staff are required to have on file at camp a record of a physical examination completed within 24 months of the time you arrive at camp. If you are under age 18, use Form 1 (parents/guardians will complete this) and Form 2 (your physician will complete this).

LTD Application Packet
If you wish to apply for the Lakeside Training & Discipleship program, you may download the application packet. It includes LTD Application Info, the LTD Application Form, the Reference Form (you will need 3 copies), the Background Investigation Forms and a health form.. If you participated in the LTD program last year, you do not need to submit written references (however you must still list references on your application form). The health form is not required as part of the application process, but you must have a physical (within 24 months before camp) if you are accepted into the program. All staff members, LTD participants, and volunteers must submit an updated health form upon arrival at camp. If you are under age 18, use Health Form 1 (parents/guardians will complete this) and Health Form 2 (your physician will complete this).
